James M. Hotaling, MD, MS, FECSM

Dr. Hotaling is a fellowship-trained urologist specializing in Male Infertility and Men’s Health. He completed his undergraduate work at Dartmouth, graduating magna cum laude with a double major in history and biophysical chemistry. He then went to Duke for medical school and completed a 6-year residency at the University of Washington, where he trained with one of the top penile reconstructive surgeons in the world, Dr. Hunter Wessells. There, he had extensive training in Erectile Dysfunction, Peyronie’s Disease and Male Infertility. Dr. Hotaling elected to pursue an additional year of training under Dr. Craig Niederberger at the University of Illinois at Chicago focusing on Male Infertility and Men’s Health. Here, he had extensive training in micro-surgery, including Testicular Sperm Extractions (TESE), Micro-dissection Testicular Sperm Extraction (microTESE), and Vasectomy Reversal. He is also one of the only Men’s Health and Infertility experts in the United States to have undergone additional training to become a Fellow of European College of Sexual Medicine (FECSM).

He is currently the highest volume Male Infertility expert in the Intermountain West and performs numerous penile implant surgeries for the treatment of Erectile Dysfunction. His practice consists exclusively of Male Infertility, Men’s Health, Erectile Dysfunction, and surgical management of Testicular Pain and Peyronie’s Disease and he has excellent outcomes.

He has over 125 publications, is funded by the NIH to study Erectile Dysfunction and Male Infertility and is regularly invited to speak at conferences all over the United States and internationally on Male Infertility, Men’s Health and Erectile Dysfunction. He has been on the faculty at the University of Utah since 2013 and is currently the medical director of the fertility integrated practice unit, the director of the Men’s Health program and a co-director of the fellowship in reconstructive urology and men’s health. In addition, Dr. Hotaling is an editor of Fertility and Sterility, the premier journal in the field and is the secretary of the Society for the Study of Male Reproduction (SSMR) of the American Urological Association (AUA) and will be president of this society in 2022. This is the premier society in the field.

My goal in working with patients is to help them achieve their fertility and/or men’s health goals with the most cost-effective and data-driven means possible. At the University of Utah Health, we seek to provide hope for even the most complex cases and offer compassionate care in a pleasant and relaxed setting that is designed to cater to sensitive men’s health and infertility issues. Further, we are fully integrated with state of the art in vitro fertilization laboratory. We see many patients with male infertility, erectile dysfunction, complex scrotal pain and Peyronie’s disease each week and strive to provide optimal care for each of these conditions.

Dr. Hotaling has research interests in the genetic and epigenetic determinants of Infertility and Erectile Dysfunction, the impact of Male Infertility on individual and familial somatic health and microfluidic sperm sorting. He has over 85 publications, numerous book chapters and speaks regularly on these topics at national and international meetings. Dr. Hotaling obtained an MS in clinical epidemiology at the University of Washington and focused on applying the tools of statistical genetics to Erectile Dysfunction.

Since joining the University of Utah, he has been continuously funded by the NIH/NSF. Dr. Hotaling serves as a principal investigator (PI) on an R01 to study the genetics of Erectile Dysfunction and an NSF SSTR Phase I grant to examine microfluidic sperm sorting. He is also a Co-investigator (co-I) on two other R01s examining the trans-generational effects of smoking and the largest study in the world on the genetics of Male Infertility. Additional funding stems from being a co-investigator on two large NICHD sponsored trials of Male Fertility, FAZST, the largest randomized controlled trial of Male Infertility ever performed and IDEAL, which examines the impact of pre-conception health on fertility. He has also cofounded three companies in the Men’s Health/Male Infertility space and won multiple national awards for research.

His work on Male Infertility has centered on establishing a tissue bank of testicular tissue which now contains over 300 specimens as well as building a large (>50,000 infertile individuals linked to >500,000 relatives) infertility database through the Utah Population Database called SHARE: subfertility health assisted reproduction and the environment. SHARE is the largest repository of epidemiologic and pedigree based information on male infertility in the United States and, likely, the world. Multiple publications from this database have helped elucidate the impact of Male Infertility on individual and familial somatic health. Additionally, he has played an integral role in complex translational genetic and epigenetic work that has sought to identify the causes of Male Infertility with a long-term goal of developing genetic and epigenetic markers for Male Infertility that will eventually lead to novel therapeutics for this devastating condition.

Clinical and translational microfluidics work has centered on novel technology to optimize sperm retrieval, sorting and cryopreservation for men with non-obstructive azoospermia or spermatogenic failure. We have developed a novel chip that, in preliminary research trials, appears to significantly increase the yield of sperm retrieval from micro-dissection testicular sperm extraction (microTESE). We are now working on refining the technology which will, hopefully, allow the sorting and cryopreservation of very low volumes of sperm for future use in ART.
